Birth/Death Certificates
How to Request Death Certificates
- We are able to issue death certificates for anyone pronounced deceased in the state of Ohio.
- Death Records are $26.50. Certified copies are required for most official uses of vital records. They carry the seal of the registrar and are on special security paper.
- Visit the Harrison County Health Department at 538 N Main St, Cadiz, OH 43907. Hours are Monday through Friday, 8am-4:00pm, but closed government holidays.
- You can obtain a death certificate online through the Department of Health or by coming into our department.
- You can print an application ahead of time for Death Certificates by clicking here.
- Normal waiting times for same-day certificates are 5 to 10 minutes.
- We accept check, cash, and credit card.

- We are able to issue birth certificates for anyone born in the state of Ohio.
- Certified Birth Certificates are $26.50, certified copies are required for most official uses of vital records, such as school enrollment, driving permit/license, and marriage license. They carry the seal of the registrar and are on special security paper.
- Visit the HCHD office at

- You can obtain a birth certificate online through the Department of Health or by coming into our department.
- You can print an application ahead of time for Birth Certificates by clicking here.

About Harrison County Vital Records
Vital records include Birth Certificates, Death Certificates, Paternity Affidavits and Burial Permits. The Harrison County Health Department maintains records of Births and Deaths occurring within Harrison County from December 20, 1908 to present.
Older records from 1867 to December 19, 1908 and Marriage Certificates, are provided by Harrison County, Courthouse, Cadiz, OH 43907. Call 740-942-2616. Prior to 1867, Ohio law did not require records to be kept.
IMPORTANT NOTICE REGARDING DEATH CERTIFICATES:
ORC 3705.25(5) requires that the social security number be redacted off of any death certificate for a person who has died within the last five years unless the person applying for the certificate can prove that they are either a, spouse or legal partner, natural or adopted child, grandchild, or great-grandchild, funeral home, veteran’s administration officer, law enforcement agency, executor or administrator of estate, etc. For a full list of relationship and what qualities as acceptable proof please call 740-942-2616.
